Payroll Alert: Fraud – Authorization for Payroll Direct Deposit

Dear School Business Professionals,
Authorization for Payroll Direct Deposit Form – All districts should be aware that an attempt at fraudulent behavior related to this form has occurred at one of our districts. See below for more information.
It has come to our attention that some districts may have placed the Authorization for Payroll Direct Deposit Form as a link on their public-facing website without a username/password. One of our districts has identified that a form was submitted to their payroll office had fraudulent banking information entered on it. It appeared exactly like a normal employee request. Fortunately, sharp eyes were with the payroll staff, it was discovered, and not processed but instead reported to our office.
We are requesting that districts exercise extreme caution with this form by password protecting it or placing on a protected district intranet.
Please be diligent in examining each form when you receive it. We highly recommend following up with employees personally to confirm that the form received was indeed submitted by the employee. If you have any questions about this, please contact the SBAS payroll team for assistance.
